Modern Movement provides the following appointment types/lengths
All visits will be completed by Michael Price, PT, DPT, OCS, CSCS
Modern Movement is an out-of-network provider. If you wish to submit to your insurance for a claim, we will be happy to provide you with a Superbill at the end of your visit. Please note that a full payment is due to Modern Movement Physical Therapy at the time of your visit. Any claims that are approved by your insurance company will be paid at a later date. We highly advise that you familiarize yourself with your insurance policy’s out-of-network benefits as they differ for each individual plan.
Initial Evaluation
$250
1-hour session with the physical therapist. This includes evaluation, review of systems, and review of diagnostics followed by manual therapy interventions and exercise prescription and education.
*Please note that this is required for all first-time patients
45 Minute PT Session
$180
30-45 minutes of manual therapy which may include joint or spinal manipulation, mobilization, dry needling, cupping, shockwave, myofascial work, or kinesiotaping. Other interventions may be deemed appropriate and will be determined on an individual and case by case basis. Manual therapy is followed by exercise prescription for 15 minutes or 15 minutes of additional manual therapy.
30 Minute PT Session
$125
30 minutes of manual therapy which may include joint or spinal manipulation, mobilization, dry needling, cupping, shockwave, myofascial work, or kinesiotaping. Other interventions may be deemed appropriate and will be determined on an individual and case by case basis.
Packages
All appointment types can be purchased in packages of 12 at a 10% discount
Appointments purchased in a package can be used at any time within 12 months of purchase.
75 Minute PT Session
$275
75 minutes of any combination of manual therapy and exercise. For example, this could be 30 minutes of manual and 45 minutes of exercise/training. This will be determined on a case by case basis and the goals of the client.
60 Minute PT Session
$215
60 minutes of any combination of manual therapy and exercise. This could be 60 minutes of manual therapy or 60 minutes of all exercise/training. This will be determined on a case by case basis and the goals of the client.
Concierge Membership
$4000
Concierge membership carries with it guaranteed access to care within 48 hours of request. Concierge membership entitles you and your immediate family members to 18 concierge visits over a 12-month period with 48-hour PRIORITY scheduling.
All visits will be completed by the owner and founder of Modern Movement Physical Therapy, Dr. Michael Price. Concierge members will receive courtesy notification of anticipated clinic closures. Members will receive 45-minute sessions. See description of a 45 Minute PT Session above.
